Your subconscious (sleeping) mind was aware of the pain and created the dream as a way of explaining the pain. When you awoke, it only seemed that the dream preceded the pain, when in fact, the pain was the cause of the dream.

How is it possible to feel physical pain in exactly the same spot and moment as when the you in your dream got injured?

when you think about that pain in your dream your brain thinks that moment is real so your nerves fell pain and so do you. The most likely explanation is that the physical pain caused the dream, rather than the dream causing the pain. For example, suppose you rolled over onto a sharp pin while sleeping. If the pain did not awaken you, it could be incorporated into a dream, perhaps of being stung by a bee exactly where you were jabbed by the pin. The physical body affects one's dreams much more often than dreams affect one's physical body.
