Nothing, it all depends on how your body reacts to dreaming, some people dream in black and white and some in color. It is the same with pain, some peoples brains are wired so when they are dreaming they can feel pain while others are wired so they can't.
Your subconscious (sleeping) mind was aware of the pain and created the dream as a way of explaining the pain. When you awoke, it only seemed that the dream preceded the pain, when in fact, the pain was the cause of the dream.
