It could mean any number of things. The first thing I would do is urge him to get a complete medical work-up, and he would absolutely need to share his lack of sexual interest with his PCP when he gets it. If his doc is unaware of the problem, he might not think to ask about or check certain things that will be helpful. There might be physiological things going on that are treatable.
