This mnemonic is specific to asking history about pain. Onset- did the pain come on fast or slow Provocation/palliation- what makes the pain feel better or worse Quality- How does it feel? Sharp, throbbing, ache, ripping/tearing, etc. Radiation- Does the pain go anywhere, or radiate to other areas? Severity- Rate the pain on a 1/10 scale, 10 being the worst pain you've ever felt Time- When did it start? (May also want to ask what they were doing at that time)
