It means that you (supposedly) understand all the rights guaranteed to you by the Constitution protecting you from self-incrimination and you are voluntarily waiving (not exercising) those rights. When you do so you will most likely either be tape/video recorded and also asked to sign a release to that effect.

If asked by police during questioning, it means you do not wish to consult with an attorney before/during questioning.

If asked during a trial, it means you do not wish to have an attorney at all.
