The most likely explanation is that your subconscious is alerting your waking mind that you are producing something nasty. The worms could represent nasty comments or thoughts about others, particularly if the worms are coming from your mouth. Or your deep mind could be distressed over something you are producing or causing to happen. What is "coming out of you" that is repulsive or harmful?
Or, on a completely different track, your subconscious mind may be aware of a disease or other unhealthy condition in your body, that you are not yet aware of consciously. If you possibly could have contracted a parasite, or some contagious disease, you might consider being tested.
