1953. Turkey, corn bread dressing, peas, sweet potatoes. It was a foil divided container, with foil over the top, tightly crimped. Later they began to seal them better. You could open your box and find your food had slid into the outer box.
This meal resembled an airplane meal. More of a snack size. Later they said the shape of the meal container looked like the television boxes of the 50's.
This was back when television was soon to enter people's homes. NBC, CBS, ABC were not in every part of the country, larger populated cities got all 3. So, to enjoy programming, just about every house had a set of TV tray tables. There are many homes of elderly who still have these. They came in sets of four, you folded them correctly and you had a stand within the 4 that were part of the living room furniture!

The word "entree" comes from the French language, where it originally referred to the first course of a meal. In American English, "entree" is commonly used to describe the main course of a meal.
The Pilgrims' first Thanksgiving dinner included potatoes, fowl, and beans. I'm not so sure they had rice.
Soup first, then salad. An exception would be if the soup was the main entree, then the salad would be served first. Then again, if the salad contains meat like chicken, then it's more like a meal and the soup should come first. Bottom line is there is no real right or wrong way. There are places in Rome where they serve the salad last ... after dinner. Yes, after dinner.
