Most crowns are made in dental laboratories by highly trained dental lab technicians. Some dentists do have the facilities to make crowns themselves though. Always ask where your crown is being made as many dentists ship them to foreign countries (such as China or Mexico) and are not required to inform you of this unless you ask. Crowns made in foreign countries are not subject to the regulations of the FDA.
