One can find an US bill of sale form at their local Department of Motor Vehicles, or DMV. You can also download and print out the form by logging onto the DMV main website.
All US states share the information in their DMV files and honor each other's court decisions.
If you are in the US, you will need to go to the DMV office with the title of the car signed over to you.

No (not that I am aware in any state of the US--the collections industry has done so for years). For the correct fee, anyone can submit tag numbers, VINs, or operator's license numbers and receive a report from the DMV (or Secretary of State). Some offices may require the Social Security Number to produce a report.
