Hi there what hapenned here is that when any thing is heated it expands and its particles get wider apart. The jam jar lid was metal and the body was glass. the cheap grade metal has a lower melting point then the glass so its particles would get wider quicker and at a lower temp . so the meal lid got a fragment larger because of the heat which allowed it to be taken off as less pressure was on it as before there was alot as it was to tight.

The heat would stay in the pot and if you don't watch the boiling water it could over-boil. Other than that, nothing would really happen.
This may depend on the recipe. Simmering with a lid on uses less heat than with the lid off. Leaving the lid on means that evaporation will be lower - but sometimes the purpose of simmering is to reduce the amount of fluid.... so this may be necessary.
