When elections are free, that means that there is an actual choice; anyone can run for office and anyone can vote for the candidate whom they actually prefer. Many countries have had fake elections to create an illusion of democracy. In a fake election, some candidates are not allowed to run and may even be arrested if they try to run, or in some cases, only one political party is allowed (communist nations have always used that system - they have elections, but there is only one candidate on the ballot) or people are coerced to vote in the way that the government requires.

I'm not sure I understand your question. Most Communist countries have been run by a dictator: there may have been "elections" but the result is already decided. Each Communist country has its own leader who is in charge of the government. Some are called Prime Minister, some are called President, some are called Chairman (of the Communist Party). But there are rarely free and fair elections, nor is dissent generally allowed; the leader of each of the Communist countries generally rules that country with an iron hand.
