When someone destroys a horcrux they destroy the part of the soul hidden within it. It is extremely difficult to do so because only magic which is unreleasable will harm it and physical damage doesn't work. It needs to be a piece of magic which breaks the bond between the object and the soul. Nothing happens to the person destroying the horcrux unless they have been in some ways under its' power, then the spell will be broken. The wizard who created the horcrux may or may not realise that the horcrux has been destroyed. It could depend on how many hocruxes he or she has created because logically, if you have split the soul 7 ways then you are less likely to notice if one is destroyed than if you had split it 3 ways because they are smaller chunks. Like is you were to split a cake 20 ways, you are less likely to notice if one piece is missing rather than 2!

The last remaining horcrux was the snake, Nagini. Neville destroyed it with the Sword of Gryffindor.
The only horcrux Harry Potter destroys himself is Riddle's Diary. He destroys it with a basilisk fang.

Harry Potter is the horcrux Voldemort never ment to make. Voldemort made 5, then when he tried to kill Harry when Harry was a baby, a part Voldemort attached to the only living left in the room; Harry Potter. So, yes, Harry Potter is the penultimate (sixth) horcrux. The last one (the seventh) was Nagini.
