The story sort of evolved over the centuries--its origin can be found in the story of Pyramus and Thisbe which appears in A Midsummer Night's Dream and goes back to ancient Rome. The story seems to have located itself in Verona, and the characters to have assumed something like the names they had in Shakespeare's play, in a story by Luigi da Porto written in 1530.
