Files are stored on a computer in such a way as to become fragmented. On a new hard disc the files are store in continuous blocks. Over time files are deleted from the computer as the hard drive becomes fuller this leaves empty spaces. New files are added to these spaces as the hard drive looks for the first empty space to store new files. Over time the files may be too large to fit into that space so they are split and stored in 2 or more spaces. They thus become fragmented. When you defrag your computer the hard drive moves these files together thus eliminating the fragments. It is then easier for the hard drive to recover the files as they are in one place instead of being fragmented.
The full word is defragment. It means to clean up a hard disk by collecting the fragments of large files to form single unfragmented files and generate a single unbroken free space. This will speed up access to the disk, as it won't have to keep jumping around to locate the next file fragment all the time.
You don't need to defrag the system reserve but if you want to anyway there is a program called auslogis disk defrag that lets you defrag system reserve

A defrag program sometimes skips the files that contain like 2-4 fragments so it won't need to make space for that file because it's to large. Some programs have the option to defrag to defrag every file without skipping one.

It would'nt take forever but yes, normally the bigger your hard drive is, the longer it takes to defrag. It also depends on how bad the hard drive is in need of the defrag.
