It's dead on arrival. It's a term used to indicate that by the time the person filling out a report (police officer, coroner, ambulance crew, doctor, etc.) saw the victim, he was dead. It's abbreviated D.O.A. It may be used in nonmedical fields as a sort of metaphor. The tow truck guy gets to the accident and the car is D.O.A.
