its when an apprentice becomes a warrior
Here is how it usually happens:
Leader:"I,(leader's name), call upon my warrior ancestors to look down on this apprentice. S/he has been trained well in your warrior code.
(apprentice's name), do you promise to uphold the ways of your Clan and protect it, even at the cost of your life?"
Apprentice:"I do."
Leader:"Then by the powers of StarClan i give you your warrior name. (Apprentice's name), from this moment on you will be known as (new name).
StarClan honors your (virtues), and we welcome you as a full warrior of (clan)."
The warrior cat ceremonies are called "Gathering" and they happen at "Fourtrees" they are supposed to be a time of peace but every now and then clans and clan leader will argue and Starclan (a place cats go when the die) shows there anger. One time they showed there anger by blocking out the moon. And another warrior cat ceremony is when you come from an apprentice to a warrior. your leader will ask if you will defend your clan even with your life and you usually say i do unless your stupid because that would mean not becoming a warrior.
In the book series, it depended on an apprentice's loyalty and readiness to take on the full responsibility for their clan as a Warrior. The leader will then determine if the apprentice is ready for the naming ceremony.
Leader: (Warrior), is it your wish to give up the name of a warrior and go to join the elders?Warrior: It is.Leader: Your Clan honors you and all the service you have given to us. I call upon StarClan to give you many moons of rest.
Thee is no ritual prayer at a Warrior death ceremony, but the cats who were friends with the warrior speak one last time to the warrior, then sit vigil for him/her that night, and in the night or morning the elders bury the body.
They become a warrior at 12 moons, unless there's something stopping them from becoming one (Ex. an injury, blindness, ect. ect.) And they would become a Medicine Cat when either the medicine cat dies, or believes the apprentice is ready to become the Medicine Cat.
A ceremony consists of an apprentice being accepted as a warrior, or a kit being old enough to become an apprentice. Apprentice to warrior - The apprentice is brought up before the Clan to receive their warrior name and rank from the Clan leader. They must say "I do" to questions asking for their loyalty and courage. The Clan leader then gives the apprentice their new name, and the new warrior will spend the night on camp watch. Kit to apprentice - The kit is brought up before the Clan to receive their apprentice name and rank from the Clan leader. They must say "I do" to questions asking for their loyalty and courage. The Clan leader then gives the kit their new name, and the new kit will move into the apprentice's den.
You have to be an apprentice first. Then your mentor will train you how to fight and hunt until he/she thinks that you are ready to become a warrior. After that the apprentice will have his/her warrior ceremony and become a warrior.
Berrypaw may not have become a warrior yet due to still being in training or not having completed all the necessary skills and assessments required to become a warrior. Other factors, such as leadership decisions or clan dynamics, could also play a role in delaying Berrypaw's warrior ceremony.

The exact words can vary, but here's the jist of it (taken from Bluestar's ceremony) Leader: [kit's name]! Until you earn your warrior name*, you will be [apprentice's name]. Your mentor will be [mentor's name]. Note: "Until you earn your warrior name" is often also said as "From this day forward"
