They become a warrior at 12 moons, unless there's something stopping them from becoming one (Ex. an injury, blindness, ect. ect.) And they would become a Medicine Cat when either the medicine cat dies, or believes the apprentice is ready to become the Medicine Cat.

In the warriors series by Erin Hunter, an apprentice becomes a warrior after completing their training and receiving their warrior name in a ceremony called the warrior ceremony. A medicine cat apprentice becomes a full medicine cat once they have completed their training and received a new name as well.
