Pancreaticojejunostomy is a surgical technique used for treating chronic pancreatitis.

The technique involves a longitudinal incision along the pancreas while the main pancreatic duct is filleted open from the head of the organ to its tail. The duct and pancreas are then attatched to a loop of the small intestine (Pancreaticojejunostomy) which is then oversewn to the exposed pancreatic duct to allow it to drain. Pain from chronic pancreatitis is either lessened or eliminated.

This procedure is preferred over a similar procedure known as 'Frey's Procedure', because a Pancreaticojejunostomy allows the pancreatic tissue to be preserved, which is critically important in patients with exocrine or endocrine insufficiency caused by chronic pancreatitis.

Pancreaticojejunostomy is also known as the Puestow Procedure.
