Usually, junk food falls in this category. They contain massive amounts of carbs, sodium and calories which don't benefit one's health.

It depends on the size of the bread. Your best source would be the nutrition guide on the bag or the nutrition guide on a restaurant's websites. I've found wheat bread as low as 90 calories for two pieces and as high as 200. It really varies.

High fat/high carb foods are quite caloric. I think you're out of luck on this one.
