A lurching heart is symbolic language saying that the heart is making dramatic movements ... it could mean several things, depending on the context. It could mean that the person is undecided in who to love, and thus the heart "lurches" one way and then another. It could just mean that the person feels so strongly about one thing or person that the heart tries to go strongly in that direction, so it is hard to stay calm and make rational decisions. It could even mean that the person is really frightened, and their heart is jumping inside because of so much adrenaline and no place to take action.
