Passing though immediate heirs or parents. I.e. a title which is passed down from father to son to grandson to great grandson etc. without going sideways through a nephew or niece. I would consider there is some ambiguity about whether mixed genders would be a direct line. The daughter of a daughter of an Earl is directly descended, but as the phrase is generally used about titles I for one tend to think of it as a line of single gender, though I suspect it need not be.
