It is not on a disc! It is on the sheet that says Halo Reach, But you will need Xbox Live.

Yes, Halo 4 comes with two discs. It has the main disc, which has the campaign and spartan ops modes, and the secondary disc which you put in the Xbox and download. Then you can take it out and play multiplayer (War Games) on the first disc again.
