Not to demean your reply but the use of French language is very precise...to say " Ca va?" is to take away the word "HOW" from the question "how are you?"...and, "Ca va bien?" translate to "Doing well? thus,the question,is actually an answer...

The individual can then invites you to do otherwise,in abdicating the use of formality,then "Comment vas-tu?" becomes appropriate.
In comparison to the simplicity (direct manner),of English,French has an appropriate word for each specific situation in which it is used.If one desires to become bilingual, to show respect and knowledge in expressing the differences in relationships status,or simply to not risk being misinterpreted ,it is always best to address someone appropriately.
