Depending on the type of worms, it could be caused by different things, but most are caused by the dog eating a flea that hosts the egg of the parasite. The dog can also get it if he eats the excrement of another infected animal. The best prevention would be a good flea prevention and cleaning up the yard (or area where you walk him).

Yes a indoor dog can get worms.Just by eating same things and for an example for dogs just meat is not good.It should be biscuits and meat or bones.
