Many women have regular cycles well into perimenopause, but they are not ovulatinga, because they don't have enough eggs left(you can find out by a day 3 FSH blood test), or the aging follicle doesn't make it into the tube. If a woman is ovulating on her own, the egg itself doesn't have what it takes to implant;therefore there is sometimes fertilization but no implantation. Sometimes the hormonal and uterine environment cannot sustain the implantation, or the rest of the pregnancy. Even with drugs, etc., miscarriage rate, if I am not mistaken, is about 50%--well beyond the standard 20% of younger women.
