"hors d'oeuvres" doen't need to be capitalzed in any way - that is, unless you capitalize the other items in the menu. If you want to capitalize it, it should be either "Hors d'oeuvre" or "Hors d'Oeuvre", the first being in my opinion less showy.

Anyway, except for "hors d'oeuvres variés" (various sorts that we don't want to list), we tend to write the name of the course, for instance:

"Feuilleté aux champignons",

"Terrine de saumon", etc..
