Regular air filters are in a housing box towards the front of the car and are simply an insert laid into the housing which protects the engine from dust and dirt from the air so that it can breathe better. Since the whole intake is housed and enclosed, it draws hot air from the engine bay to feed the engine it's oxygen.
Installing a cold air intake constitutes removing the whole housing assembly and unbolting it from the engine bay. It is replaced with a pipe connected directly to the throttle body that leads down by the driver's side wheel well. On the end is the air filter. This draws cooler air in from outside the engine bay. Cold air is favorable because it is more dense, which will help the combustion of the engine, in turn producing more power. The engine can breathe easier as well, in turn working less hard, which (in theory) gives you slightly better MPG.
BEWARE: Installing a CAI puts your engine at risk of hydrolock.
The difference between a fresh air and a regular intake on a vehicle is the location of the inlet. Fresh air uses and inlet at the extreme front of the vehicle, while a regular intake can be anywhere and pull in heated air from the engine.
