There are two kinds of doctors: MD, or medical doctors, veterinarians, dentists, chiropractors and so forth, as opposed to PhD, JD, DSW and others that are not medically-related degrees, but which signify a presumed expertise in some other field of study, such as education, law, divinity or English, among many.
In addition to that, the issue of academics could include, in the case of your question, a medical Doctor Who teaches but does not practice (academic) as opposed to a practicing physician who sees patients. (Most teaching MDs are both.)
