Well from my experience
not enough salad bars, in fact, no salad bars at my school. too many bags of chips and crappy food that's bad and makes you gain weight.
another thing was how its made, how sanitary it is. some of the food sits out which can be contaminated if the kids don't use the tongs or spoons provided. many kids grab with their hands, decide they don't want it and put it back.
