What is chitin made of?
Chitin is a polysaccharide (polymer) of N-acetylglucosamine, which is a derivative of glucose. It's chemical formula is: (C8H13O5N)n ...where "n" is the monomer length of the polymer. Chitin is similar to the polymer cellulose in terms of function in biological organisms (primarily as a structural polymer), but has a completely different chemical structure.

What is the chemical formula of the monomer and the polymer?
Those are two general terms, so there is no chemical formula. A polypeptide is for instance a polymer where it consists of amino acids (monomer). Same with polyester which consists of esters (monomer). Being more specific, ethylene has the formula CH2=CH2 and when polymerized into a polymer would be (--CH2-CH2--)N where N is a very large number. The same principle applies to many monomers and polymers.
