I believe it's the same as "against all odds," meaning that the probability of the event occurring was low (it was unlikely to occur), yet it did so anyway. Usually the implication is that extraordinary deeds or qualities (determination, preparation, virtue, etc) yielded an extraordinary result, whereas an ordinary result is dictated by probability, or chance.
