Perhaps leprosy would be considered the most gruesome. The person skin becomes so deformed and folded that they are said to look like lions.
The Black Death (bubonic plague) can result in pustules covering large areas of the skin. This however is only an outward symptom and death results from other effects. And you will not see this except in Africa today.
Another"gruesome" is necrotizing fasciitis, in which huge areas of skin and muscle can simply die and fall off. This is commonly called "flesh-eating bacteria" and is not as communicable (infectious). It typically affects those with weakened immune systems.
