It is done by heart that means you intend to fast the days of Ramadan. Getting your late meal (Souhour in Arabic) is also by itself niyat to fasting.
It is simple announcing that you intend to perform Hajj of the House of Almighty Allah, to fulfil the fifth pillar of Islam. I intend to do Hajj only to please Almighty Allah.
