(Matthew 26: 26-28) In the Last Supper Jesus told his deciples the bread was like his body and the wine was like his blood. His body was broken to pay for our sins, (hence, the "breaking of bread").

As far as whether or not it is unique to Christianity, I'm pretty sure Christians are the only ones who normally perform the ritual.
