You must first petition the court where the order was originated. You can get a petition form atwww.nycourthelp.gov. They actually walk you through the form process. You then need to have the petition notarized and send it to the family court. in about two weeks you will receive a copy of the petition that was sent to the custodial parent and you will also receive a court order to appear at family court. You will also need to send them a Financial Disclosure form also available on-line at www.nycourts.gov (which too must be notarized), your latest tax return, your latest paycheck stub, and any other documents you wish to submit The rest is up to the court.
