As the Sun came into life, the lighter elements (hydrogen and helium etc) were "blasted" away from the Sun, while the heavier elements (iron and rock) stayed closer to the Sun. These heavier elements coalesced into the inner or terrestrial planets, while the lighter elements, now further from the Sun, passed, what is known as the ice line [See related link]. The reduction in temperature allowed the lighter elements to condense into solid particles and the gas giants or Jovian planets began to form. Many of the exoplanets that have been found are too close to their star to have formed in this manner but it is believed that they were formed outside of the ice line and have subsequently been drawn closer to their host star.
The gas planets are much more massive than the terrestrial planets and therefore have stronger gravity. It is believed that as they formed many of their moons formed around them much like the planets formed around the sun. Other moons are likely captured asteroids and comets.

Terrestrial planets are made of denser materials than Jovian planets. The planets were primarily formed from the left over disc of dust that orbited our sun when it first formed. The heavier elements were draw closer to the sun by gravity and the lighter ones flung further outward by their high orbital speed.

Yes and no. It was the same material initially when the solar nebula first condensed, however then the sun ignited and cooked the volatiles (e.g. gases, water) out of the material nearby and the solar wind carried those volatiles outward. The material further out retained the original volatiles. The volatile depleted material then formed the four terrestrial planets and the undepleted material then formed the four gas giant planets.

Dependant on how the debris from supernovae is moving, the proportions of certain materials present, and the initial conditions of solar systems forming different combinations of terrestrial and Jovian planets are formed. Terrestrial planets are comprised of heavier particles (ie. of higher relative atomic mass) meaning they have stronger gravity which leads to higher density planets than the gaseous Jovian planets. A Jovian planet can be thought of as a failed star - it has all the elements needed to fuel nuclear fusion, but not enough gravity to attain the nuclear activation energy required. So the Sun started as a large cloud mostly comprising of gases, and terrestrial planets were formed from clouds mostly consisting of heavy solids.
