Inside the cave Hira, the prophet received his first Revelation, which is a letter Al Alaq ayah 1 to 5, which sent the Prophet to read, same as first ayah: Iq'ro 'Bismi Rabbikalladzi Kholaq' which means: Read in the name of your God who created.
The first person to write Bismillah al-rahman al-rahim was Prophet Muhammad (peace be upon him) when he sent letters to different rulers during his time. The phrase Bismillah al-rahman al-rahim means "In the name of Allah, the Most Gracious, the Most Merciful" and is often used to start a new task or activity.

No. Muhammad, peace be upon him, did not know he was The Messenger of God until Gabriel came to him in the cave of Hiraa at the age of fourty. When exiting the cave, after Gabriel delivered the first words of The Qur'an - "Iqraa Bismi Rabbik Allathee Khalaq . . .", he saw gabriel on the horizon. Gabriel then said to him " I am Gabriel; and you are Muhammad the Messenger of God". Though, there is also evidence through the writings of the scholars that, as he oftentimes made his way to and from the cave, he would hear voices coming from the rocks and trees saying "Peace be upon you, oh Messenger of God". Though Muhammad, peace be upon him, had an unusual experience at roughly the age of six, when two angels in the form of men came to him and extracted a small clot from his heart, he did not know what to make of the incident. Peace and blessings be upon him.
