For a standard switch, it doesn't matter. If it's a 3 way switch, "hot" is connected to the "common" connection.
The standard height is around 12" to 15" the bottom of the box above the floor but for the disabled you can install them higher but no more than 48" above the floor. Pick a height that will work for the disabled and install all of them the exact same height.
You would need to purchase a duplex switch. As for the pilot light look for a duplex switch that has a neon pilot light that is incorporated into the handle of the switch. When the switch is turned on the switch's handle will illuminate.

The standard height they are placed is no more than 48" above the floor. I mount my switch boxes with the top of the box 48" above the floor. You can install them lower if you wish but no lower than 15" above the floor.
For a light switch in the home four foot to centre is a reasonable height. Some contractors set the outlet box at four foot to the bottom of the outlet box and other contractors set the box to four foot to the top of the outlet box. There is no code rule that stipulates the exact height that the outlet box must be set to.

It is located next to the shifter wher the ECT PWR control is. Please be mindful that Air Ride suspension (height control as you call it) was an option not standard so not all models have it and also it will only switch to LO when you exceed 85 mph. LO is not an option just NORM and HIGH. MC
