There are two types of tests for T4.
Total testing is antiquated and no longer useful since the Free T4 test was devised. Total testing tells you how much total amount of T4 is in your blood. This includes the T4 that is bound to other proteins and hormones that cannot be used by the body, so is an inaccurate test.
Free testing is the current gold standard, most accurate way to test T4. It shows how much of the T4 is floating freely, ready to be used by the body by conversion to T3.
It is a test for hypothyroidism and evaluates thyroid function.
The test is generally performed when a lump or a nodule is detected in the thyroid. The test may also be ordered if the thyroid gland is enlarged and the cause is not apparent. The biopsy is usually a test for thyroid cancer.

Unless your doctor asks you to fast for your thyroid test(s), usually you do not need to for a blood test. Some thyroid tests, such as a thyroid uptake scan, requires you to stay away from eating certain fish (usually shellfish) prior to and shortly after the test is conducted.
