Answer: No it doesn't. Unfaithful assumes that the husband doesn't want her to. If the husband is ok with it, then it doesn't harm the marriage. I would be sure the husband is actually ok with it though, alot of people think they are ok with something like this until it actually occurs.
Answer: I guess some people would consider it appropriate, some not. Those who don't consider it appropriate would consider that sexual intercourse should only occur between husband and wife. In turn, there can be several reasons for that: reduction of sexually transmitted diseases, and religious considerations are some of them.
