Assuming you're referring to the tool necessary to be able to access the Forest Temple, head to Kakariko Graveyard. Démpe the gravekeeper has long since passed away; this opens up a new 'minigame' for you to play, simply by keeping up with the ghost of the gravekeeper. His grave is located on the west side of the cemetery, with flowers in front of it. Pull back the headstone to reveal a hole.
If you manage to keep up with Démpe, you'll be rewarded with the hookstot, allowing you to enter the temple where you must save Saria. ;)

1) Cuccos that are on the loose in the village. If you find them all and throw them back into a pen near the graveyard, you get an empty bottle.
2) The Lens of Truth. It's located at the Bottom of the Well, and you have to know the Song of Storms and play it in the Windmill to get there.
3) Gold Skulltulas. You get their tokens and get rewards at the Skulltula House in the village based on how many tokens you have.
