What kind of engine and transmission? Could be a faulty Idle Air Control if injected or a need for idle speed adjustment for a carburated engine. If the transmission has a locking torque converter, it may be stuck in locked mode instead of waiting for the proper high gear and speed conditions.

My 91 Dakota gets 14.5 city and about 18 highway. 3.9 V6 throttle body. 4 speed auto. 2WD extended cab.

my 91 with a 3.0 and almost 300,000 actual miles gets 15 mpg around town and about 18 on highway. mine is the 2x4 model with auto trans.

I have a 1991 NPR with a 12' service bed running at about 10,000 lbs and get 16- 18 mpg, a newer model 14' box truck should get about 12-14 mpg
It came from the factory with R12 Freon. It may have been converted to R134a. If so there should be a sticker near the compressor. If it has not been converted, I suggest you do so, as the price of Freon is prohibitive.
