Fluoride (f) replaces the hydroxyl group in the outer enamel of the tooth when applied topically making the enamel chemically harder. Hoewver, it doesn't reduce tooth decay, it just "hides" it as decay seeps through the micro-spaces tightly bound rods micro-spaces that make up the enamel. Whenn the first fluoride studies were done, they didn't have the advantage of electronic tools such as the diagnodent which "sees" cavities that the eye doesn't see and which are still too small to show on an x-ray.
