That's 3-4 times the mass of our Sun; I would say that such a mass getting that close to Earth would move Earth out of its orbit. Getting so close would also result in dangerous tidal forces.
That would depend on the mass of the black hole, and how close it came. A black hole the size of a star, a few light-years distance, would not be any more dangerous than a star at the same distance.

Black Crypt happened in 1992.
Black July happened on 1983-07-24.
Black holes don't reach out and grab things that happen to be passing by. Outside of the hole's "event horizon" it has the same influence as any other object with the same mass. Other bodies that pass a black hole at a distance at which they're moving slower than escape velocity will settle into orbit around the hole.
Please specify (a) a correct number, (b) a unit.
The nearest know black hole is at a distance of several thousand light-years. There are probably black holes closer by, but they are hard to detect, if they don't happen to be part of a close binary star system. The nearest know black hole is at a distance of several thousand light-years. IF? If it were moon size it would be more massive than the sun, therefore the Earth (and the moon) would be crumbled into dust by the tidal strain, all the inner planets would be thrown completely out of orbit, the orbits of the outer planets would be severely distorted, and the sun's orbit around the galaxy would be markedly shifted.

The Black Hills of South Dakota are so named because of their appearance. Seen from a distance, the Black Hills appear black due to being covered in pine trees.
