First, report it to your supervisor. Second, ask the supervisor for an Incident Report. Write out the ENTIRE incident, even if you need extra paper to write it all. Include ALL details. This document can be used to help OR to hurt you in a worker's comp claim. Do not write opinions or conclusions. Do NOT write your emotions or anything nasty you thought. Write only FACTS as you saw it. Do NOT make any threats toward your employer, or say things like "I expect the company to pay me for my medical bills" etc.
For example, one made after an employee returned to work after being hurt on the job:
At approx 12:15pm May 17, 2015 I had just finished mopping the floor outside the restrooms. I observed a child near the front door repeatedly opening the door and swinging on it. I advised the mother this was unsafe. She replied, "Leave us alone!" I turned to go report to my Supervisor that the child could get hurt. Just as I turned, the child ran by me, and slipped on the wet floor. I instinctively reached my right arm out toward her to catch her or block her fall so she would not hit her head. As a result, I slipped, my body twisted around, so I fell on my left hip, elbow and side, and felt immediate pain. Just then, the child's mother came up, screaming at me. I was still on the floor when the child's mother, wearing hiking boots, kicked me in the ribs, which sent a searing pain into my back. The child had landed so her head was on my forearm. Her mother grabbed the child by one arm, pulling her across the floor. The child was still screaming. Mrs. ___, Supervisor, arrived on scene. Two co-workers, (full name and full name), helped me to my feet and to a chair put beside me. It hurt to take a normal breath. My hip was throbbing. My elbow was bleeding. Manager, Mr ____, called an ambulance to check the child and me. While sitting there, when the manager left the area, the mother began striking the top of my head with a food service tray and slapped it against my right ear and right shoulder. She continued to drag the child, age about 3-5 yrs old, by one arm. Other customers became upset, yelling foul names at me and threatening me with statements like "__" and "____". My co-workers stood in front of me to block the upset customers. During this the ambulance arrived. They called the police. (See police report I made.) I was transported to the E.R. (see hospital report).

[So, NEVER while commuting to work.] The injury must also happen within the scope of the worker's assigned duties. Get injured fighting a coworker in the workplace is not a work related injury. The injury must be diagnosed by a doctor, not merely claimed by the worker.
