A microorganism called Plasmodium falciparum (a parasitic protist) causes the most common strand of the disease. There are other variants in Asian and southern American countries that cause similar symptoms and are caused by closely related species of the genus Plasmodium.
The parasitic protist infects people through the bite of a mosquito, and therefore the mosquito acts as a vector, carrying the disease without actually causing it. The mosquito species vary in different locations but the relationship is quite specific i.e., each mosquito carries its own strain of Plasmodium.
malaria is caused by the bite of a female mosquito. Female mosquitoes are silent, so they are not easy to spot. When a female mosquito bites an infected person, a little bit of blood is taken, which contains malaria parasites. These parasites will develop in the mosquito, and after about a week or so, when the mosquito bites once more, the person bitten will be injected with the parasites via the mosquitoes saliva. After a while, malaria will start to come, leading to headaches and fever and sometimes coma or death.

The five types of mosquitoes responsible for malaria are: Plasmodium falciparum, Plasmodium vivax, Plasmodium knowles, Plasmodium ovale and Plasmodium malariae. The mosquitoes infect humans by bites and withing weeks to months, the grow and multiply in the body, eventually causing symptoms which can include fever, coma and death.

How did malaria become a disease?
I don't think that anyone can say for sure, but as far as I know malaria and certain species of mosquitoes evolved together. From what I know many scientists believe that malaria was once single celled protists that once lived in standing water like swamps etc. They origionaly made energy using photosynthesis much like algae and plants. There is evidence of this in modern day malaria parasites. So basically the story goes that somehow a mosquito ingested this organism, and the organism mutated into a blood fluke that lives, and reproduces inside blood cells. It may be that a female mosquito ingested this organism while it was still in it's original non infective form. The organism made the mosquito kind of sick so it regurgitated it back into it's host while feeding. Once inside the host the organism had no viable way to survive. So perhaps a mutant form of this organism managed to survive by invading the blood cells of the mosquito's host. It eventually changed further to become the successful parasite that it is today. I hope this gives you an Idea of how the malaria parasite may have come about.
