If the person is under age 18, no I.D. is required, so just provide the person's legal first and last name. NEVER NEVER use a fake name, or a nickname, because name changes are usually not allowed. Very often you can't even change the name in an unpurchased reservation, such as if there is more than one airline booked in the itinerary. Middle names and titles etc do not belong on a ticket, and most airlines do not put them on tickets. At the time of purchase the airline must collect some information for the TSA's "Secure Flight" database. You'll need the passenger's FULL legal name, first, middle and last. Also their sex, Male or Female, and their date of birth (day, month and year). Phone numbers are requested, but not required. If you don't give them your phone number, they may not be able to notify you of any flight schedule changes that occur.
