In the ninth century, better known as the Dark Ages, these fortified structures were built by nobles in Europe and the Middle East. They were build in a fast manner, just digging a circular ditch to serve as a moat, with a tower built in the middle.
They were damp for several reasons. They were usually built in damp climates. The middle ages was an era of unusual cold for Europe. They lacked central heating to help dry the damp, and they were built of (leaky) stone.
They were located in many places. One of the major places they were located is Europe. They were located there for protection of the people.

The Middle Ages are usually dated as lasting from the 5th century to the 15th century in Europe. Any house built in that time and place would be medieval.

The most powerful ancient country ever is Iran! For thousand of years, Iran had been able to have a stable empire from very west to the east of the world. And It is still called with that ancient name which makes it the oldest established country! I would have to say Greece. When Alexander the great conquered Europe Greece was the biggest country and had the most powerful army in the world The Roman Empire because it built modern Europe.
I'm guessing that it could've been built in Southern Europe.
