You have to be very careful. If you are this persons friend, then talk to him/her. See if they have been under any kind of stress or if something is wrong. It also depends if this person is at school, young or older, and so on. The best thing to do is ask for help. If you care about this person you need to talk to his/her parents, school teachers, ect. It could be nothing (simple depression or a break up which could wear off in time) or it could be something very bad. If you think something might be wrong, talk.
Ask them how/why they feel that way. Also if it sounds like suicide or self harm either contact someone (like their family, a close friend) or go to where they are, comfort them and talk to them. Suicidal people need help and need someone to talk to.
