(well this is a pretty big moral dilemma...)

you have two options:

respect the doctor/patient confidentiality (i.e. keep it a secret between the two of them)

OR

alert the appropriate authorities for the situation.

i suggest you gauge how serious this situation is and act accordingly (i.e. if its something like murder tell the authorities, but if its something small keep it confidential)

personal note: sorry i cannot help any further, but in the end its the decision of the therapist what action he/she should take as only he/she knows what's happening... (i need more details for a better answer)
